Adding Constant Value to Form Field

Hello friends, below is how I can add fixed data to a form value. For example, if the user enters 1111 on the screen, I want to add data to the beginning and end of this value as (files/data/$value.pdf). I want it to post in this way. My code is as follows. Thank you in advance.
