Error in script installation

Good evening, when I try to set up the Bedesk script and complete the MySQL information, I proceed to the page where I need to enter admin details. After filling out the information here and clicking on 'Complete,' I receive an error similar to the one shown in the image below. What could be the reason for this issue?
