Is It Necessary to Establish a Company for Adsense Tax Payment?

I have been a blogger for a long time, but since 2015 I have had an AdSense account that earns money from YouTube. I withdrew the balance from that account and now I only generate income from the website. I haven't entered tax information because I read that it should be under a company name, but am I wrong? Is it enough to just fill out this form?
